I, , understand, have read and completed this questionnaire truthfully. I agree to notify my esthetician and staff via email to [email protected] of any changes to the above information. I understand that any treatments I receive here are voluntary and release this institution and/or the esthetician from liability and assume full responsibility thereof. This includes any and all services provided today AND/OR FOR ANY FUTURE APPOINTMENTS including, but not limited to, Dermaplaning, chemical, peels, Lash Lift & Tints, Brow Lamination & Tint, and all services not mentioned above.
